Initial appointment forms

Please print, review, complete and bring the Intake, Informed Consent, and Credit Card Authorization forms to your initial appointment. If you have the need to authorize individuals or agencies to release information between this provider you should also complete the Release Information form.

 If you do not have access to a printer you should plan to arrive at least 30 minutes early for paperwork.
